To obtain a copy of a birth certificate
When babies are born at Lodi Memorial Hospital, LMH completes the birth certificate for the newborn and files it with the county where the mother lives. By law, the hospital cannot keep a copy of the birth certificate.
To obtain a certified copy of a San Joaquin County birth certificate, an application form must be completed and a sworn statement must be signed. Phone requests for copies of birth certificates are not accepted by the county. There is an $18 fee, and ID must be provided.
For children less than two years old
