ABOUT THE LODI MEMORIAL HOSPITAL HOME HEALTH AGENCY

If you or someone you know is dealing with an illness or injury, the Lodi Memorial Hospital Home Health Agency's professional staff can help. 

Our team of registered nurses, home-health aides, physical, occupational and speech therapists and social workers visit and care for people where patients are most comfortable - in the home.

Our services are available 24-hours a day, seven days a week.


HOW WE CAN HELP

Registered nurses
Our staff of registered nurses with hospital experience is specially trained to provide:

  • Patient monitoring / new diagnosis
  • Physician reporting
  • Patient and family instruction
  • Wound care / wound vacs
  • Injections
  • Catheter care
  • Blood drawing
  • IV therapy / PICC lines
  • Medication instruction
  • Colostomy care
  • Diabetic instruction
  • Help with pain management
  • End-stage care
  • Multiple medication education
  • Nutrition

Home-health aides
Certified home-health aides, under the supervision of a registered nurse or therapist, can assist with:

Personal care
Light housekeeping

Social workers
During the course of an injury or illness, you may experience emotional, spiritual, family or financial stress.  Our medical social workers can assess your needs and strengths and help in these areas:

  • Finances
  • Family problems
  • Coping with stress
  • Referral to community resources
  • Coping with a terminally ill loved one

Physical, occupational and speech therapists
These registered therapists can assist in the restoration of physical function and rehabilitation through planned exercise and treatment programs to help promote independence at home.  Services include:

  • Strengthening exercises
  • Gait training
  • Home-exercise programs
  • Post-surgical care
  • Daily living skills
  • Speech-disorder therapy

Ancillary services include:

  • Respiratory therapy

WHO USES HOME HEALTH CARE
The heath-care needs of some individuals can be met at home. Our team of professionals can work with patients, doctors and families to provide the care and support needed at home. We assist:

The homebound, but not necessarily bedridden
Those in need of skilled, intermittent nursing or therapy
Residents within a 30-mile radius of Lodi
Patients with a doctor's order for care

Those with special health-care needs not listed above may call to discuss further assistance.


FEES
Services are reimbursed by Medicare, Medi-Cal, private insurance or private payment.

Emergencies
Call 209/334-3411 after hours, and ask the hospital operator to connect you to the home health nurse. Services are available 24-hours a day.  During regular business hours, call the home-health office directly.
